一例门静脉积气的手术麻醉管理

摘要：背景：患者男性，58岁,因3h前饮酒后“腹痛3小时”入院，伴恶心、呕吐，无腰背部放射痛，无畏寒、发热，无腹泻、黑便，无咳嗽、咳痰等；一般情况欠佳，神志清，痛苦面容。CT示：肝内多发积气（门静脉积气？）；待排血运性肠梗阻或肠道炎症坏死可能。诊断为“腹痛；肠坏死？”。既往“高血压”史，否认“糖尿病、冠心病”史，有胆囊手术史。方法：拟在全麻插管下行“开腹探查术”。结果：手术探查发现小肠节段性坏死，行“小肠切除，肠吻合术”。转归：手术顺利，患者康复出院。
