Research on the “community - elderly care station - family” system based on the chronic disease health risk management model

Abstract: Community chronic disease management and home-based elderly care have risen to the national strategic level. At present, the three-level medical system based on community health service stations plays a positive role in promoting the health management model. In view of this reality, this paper proposes to establish a "community-elderly care station-family" system to expand health risk management model, which aims to analyze the necessity and advantages of community elderly care stations based on the health risk management of chronic diseases, and put forward the health risk management countermeasures of community elderly care stations through the surveys of some elderly care stations in Beijing and expert questionnaires, certifies the rationality and feasibility of the "community-elderly care station-family" system in running health risk management, creatively proposes the idea of daily sustainable development of health risk management, and designs the process and market-oriented operation scheme of community health risk management.
