摘要:本研究深入剖析中-重度主动脉瓣狭窄(AS)患者并发心房颤动(AF)的潜在机制与临床意义。伴随全球人口老龄化加剧,AS 的发病率显著攀升,退行性病变已成为其主要病因。诸多研究显示在中-重度AS患者中,血流动力学的显著改变致使左心室后负荷急剧增加,进而引发左心室向心性肥厚。与此同时,神经内分泌系统的激活,尤其是肾素 - 血管紧张素 - 醛固酮系统(RAAS)的过度活化,促使心肌纤维化,并激活交感神经系统,显著增强了 AF 的易感性。尽管目前对这一领域的机制和临床意义已有一定程度的认识,但仍存在诸多亟待解决的关键问题。未来研究应着重从分子生物学层面深入探究其发病机制,挖掘潜在的治疗靶点。借助机器学习等前沿技术,有望实现对高风险患者的早期精准识别,从而制定个性化的防治策略。
